During this follow-up visit we will discuss ongoing concerns and identify your priorities moving forward. We will spend time reviewing your questions and concerns. We will review any pertinent lab results or imaging. Together we will decide on the best next steps and create a collaborative plan-of-action.
Cancellation Policy
To cancel or reschedule please contact us at least 24 hours in advance. If you cancel with less than 24 hours notice we reserve the right to charge a $50 fee.
